ReVive Center is seeking a part-time Peer Navigator to join our street outreach team. The Peer Navigator is a person with lived experience of homelessness who is ready to share their expertise in supporting others to transition into housing.

ReVive Center is seeking a full-time Clinical Case Manager to join our street outreach team. The Clinical Case Manager is responsible for providing direct clinical and case management services, including: intake and assessment, collaborative development of individual service plans, crisis intervention, individual counseling and therapeutic group activities as appropriate to support participants in meeting their goals.
